HIPAA Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL AND HEALTH-RELATED INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Commitment to Your Privacy
Faircare Insurance & Advisors (“Faircare,” “we,” “us,” or “our”) is required by law to maintain the privacy of your
Protected Health Information (“PHI”), to provide you with this notice of our legal duties and privacy practices with
respect to PHI, and to notify you following a breach of unsecured PHI. We are required to abide by the terms of
this Notice for as long as it remains in effect.
How We May Use and Disclose Your Health Information
We use and disclose PHI you provide to us — such as information collected through Medicare, health, or life
insurance intake and enrollment forms — for the following purposes:
• Plan Administration and Enrollment. To help you apply for, enroll in, or maintain health, Medicare, or life
insurance coverage, and to communicate with insurance carriers on your behalf for these purposes.
• Payment. To facilitate premium payment, billing, and claims-related activities connected with the coverage
you are seeking or maintaining.
• Health Care Operations. For quality review, agent training, business planning, and other internal
administrative activities necessary to run our insurance-advisory business.
• Business Associates. We may disclose PHI to companies that perform services on our behalf, such as form-
hosting, consulting, and administrative-support vendors, provided they agree in writing to appropriately
safeguard your information under a Business Associate Agreement.
• As Required by Law. We may disclose PHI when required to do so by federal, state, or local law, or in
response to a valid legal process.
• To Avert a Serious Threat to Health or Safety. We may disclose PHI to prevent or lessen a serious and
imminent threat to the health or safety of a person or the public.
Other uses and disclosures not described in this Notice will be made only with your written authorization. You
may revoke that authorization in writing at any time, except to the extent we have already relied on it.
